Dog Grooming Agreement and Waiver
Agreement Terms:
1. Services
Groomer agrees to provide grooming services for the Owner's pet ("Pet") as requested and described in the Owner's service request.
2. Acknowledgement of Risk
The Owner acknowledges and agrees that:
a. Grooming activities may involve inherent risks, including but not limited to cuts, nicks, scratches, allergic reactions, and stress to the Pet.
b. Groomer will take all reasonable precautions to ensure the safety and well-being of the Pet during grooming.
3. Medical Conditions and History
The Owner agrees to disclose any medical conditions, allergies, or behavioral issues that the Pet may have before grooming services are provided. Groomer will not be held liable for any adverse reactions or incidents resulting from undisclosed conditions or issues.
4. Vaccination and Health Requirements
The Owner certifies that the Pet is up-to-date on all vaccinations and is in good health. Groomer reserves the right to refuse service if these conditions are not met.
5. Emergency Medical Care
In the event of an emergency, Groomer is authorized to seek veterinary care for the Pet at the Owner's expense. Groomer will make reasonable efforts to contact the Owner before seeking such care.
6. Release of Liability
The Owner hereby releases, discharges, and agrees to hold harmless Groomer , its employees, agents, and representatives from any and all claims, damages, or liabilities arising out of or related to the grooming services provided, including any injury or damage to the Pet.
7. Photography Release
The Owner agrees that Groomer may take photographs of the Pet before, during, and after grooming for promotional or educational purposes.
8. Payment and Cancellation Policy
The Owner agrees to pay for all grooming services at the time of service. Groomer reserves the right to charge a cancellation fee for appointments canceled with less than 24 hours' notice.
9. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State the Groomer resides in.
10. Entire Agreement
This Agreement constitutes the entire agreement between the parties and supersedes any prior or contemporaneous understandings, whether written or oral.
