- I certify that my dog is in good health and has not been ill with any communicable condition in the last 30 days.
- I agree to provide proof of current vaccinations as listed above.
- I understand that if my dog is found to have fleas or ticks, treatment will be applied, and I will be charged accordingly.
Liability:
- I agree to hold the boarding facility harmless for any injury, illness, or death of my dog while under their care.
- I understand that the boarding facility will take all necessary precautions to ensure the safety and well-being of my dog, but I acknowledge that accidents or illnesses may occur.
Emergency Care:
- In the event of an emergency, I authorize the boarding facility to seek veterinary care for my dog at the nearest veterinary clinic. I agree to be responsible for any costs incurred.
Behavior:
- I certify that my dog has no history of aggressive behavior toward people or other animals.
- I agree to be responsible for any damage or injury caused by my dog while in the care of the boarding facility.
Payment:
